Physical, Medical or Special Notes to Guide us:
IMPORTANT MEDICATION INFORMATION:
If your child is likely to require any medication while they are away, (Paracetamol, Ibuprofen etc) then the required medication that you would want your child to take will need to be put into SEPARATE containers/envelopes in the ORIGINAL PACKAGING AS DISPENSED BY THE PHARMACY and be CLEARLY MARKED with the following information:
Name of Child
Name of Medication (eg Calpol)
The required dose
These containers/envelopes should then be put in ONE envelope/container and SEALED. Please ensure that your child's name is clearly marked on it.
Please send these into school before THE END OF SCHOOL MONDAY 7TH MARCH 2022 so that we can have everything ready.
Medication will be collected and kept by one member of staff. Children will NOT BE allowed to sore/administer ANY medication themselves except inhalers.
We are only able to give your child the medication that YOU PROVIDE FOR THEM. If you do not send any in then we CANNOT give them anything unless we are able to contact you first.
We acknowledge that you may need to make the decision to send medication at the last moment, if this is the case please inform the school office as soon as you are aware.
If your child will require travel sickness medication for the outward journey, you will need to administer the medication on the morning of departure. Please send the medication required for the return journey into school as directed above.
If your child uses an inhaler - a spare refill MUST be included.
PLEASE ALSO COMPLETE THE SEPARATE 'PARENTAL CONSENT FOR SCHOOL STAFF TO ADMINISTER MEDICINE' FORM FOR ALL MEDICATION YOUR CHILD MAY NEED. THIS WILL BE SENT HOME AS A PAPER COPY SHORTLY.
Thank you for completing this form.
