OWN Trust Photography and Filming Consent Form

Consent

Parents and carers: please read and discuss this consent form with your child and then fill it in and return it to the school office. Please read the consent and understanding statements carefully, tick the corresponding box and sign to confirm your decision. These permissions will be used for the duration of your child’s time at the school, but should you change your mind with regard to these permissions please contact the school office.

You will be reminded on an annual basis if you wish to change the permissions.

Childs Full Name* 1. I confirm that I have read OWN Trust and Orton Wistow Primary School's Photography and filming guidelines.* 2. I give consent to my child's photograph being used within OWN Trust and Orton Wistow Primary School for internal display purposes. 3. I give consent for photos of my child to be used within printed publications produced by OWN Trust and Orton Wistow Primary School (eg. newsletter, brochure) which may also be used online* 4. I give consent for photos of my child to be used on the OWN Trust and OWPS websites, e-newsletters and social media* 5. I give consent for photos of my child to be used by the local media for school activities (eg. newspaper, online news)* 6. I give consent for photos to be taken of my child by external photographers for individual and class photos and I DO understand that the proofs may be sent to other parents in the class (in the case of class photos) and that the external photographers will also retain a copy on their system.* 7. I give consent to my child being videoed internally for education/learning purposes at OWN Trust and Orton Wistow Primary School* 8. I give consent to my child being videoed for use on OWN Trust and Orton Wistow Primary's School's websites, newsletters and social media about learning, school trip, class activity.* 9. FOUNDATION STAGE ONLY - I give permission that some photographs are used for your child's Online Journal 'Tapestry'. Some of these photographs may be available for other parents to see, eg. Group activity photographs.* 10. I give permission for my child to be part of audio/television/radio broadcasts when the school is in the news. e.g for a local radio item.*

Understanding (please select as appropriate)

1. I understand the potential risks associated with the use and distribution of these images as outlined in the guidelines.* 2. I understand that how these images or videos will be stored within the organisation and how long for, as outlined in the guidelines.* 3. I understand that if I withdraw consent for my child's image to be used or shared in the future, it will not be possible to remove images that have already been published or distributed.* 4. I understand that I must gain permission before sharing photographs/videos of other people's children on social media or that I must not share photographs/videos taken during OWN Trust and Orton Wistow Primary's School's events on social media.* 5. I understand that if I share images of my child with friends and family, I should check the privacy settings of my social media account first to understand who else will be able to view these images.*

OTHER PERMISSIONS (please select as appropriate)

1. SUPERVISED TRIPS: I give permission for my child to go on spontaneous supervised trips around the local area* 2. EXTERNAL COMMUNICATION: I give permission that educational letters/emails/pamphlets can be sent to you from third parties e.g. Sports Club information, holiday activities, Term Time Magazines, information from feeder secondary schools etc* 3. FOWS: I give permission that my child's first name and class can be shared with Friends of Wistow School (FOWS) for fundraising distribution purposes only, e.g allocation of raffle tickets etc 4. FOWS: I give permission for my child to be given an Easter egg (or alternative for children with allergens), or a Christmas selection box from FOWS.* 5. CLASS SHARING FIRST NAME: I give permission that my child's first name can be shared with other parents in their Year group for the distribution of birthday sweets or Christmas cards.* Name of Parent ( Digital Signature)* Date*
